    What the Sodium test tells you

    Sodium is the dominant mineral in the fluid outside your cells, and it is what sets the pull that decides how water is distributed between the blood, the spaces between cells and the inside of cells themselves. Nerve and muscle signalling depend on the sodium gradient across cell membranes, and blood pressure and blood volume are closely tied to how much sodium the body retains. It is measured in almost every routine metabolic panel, and it is one of the most frequently abnormal results in medicine.

    The single most useful thing to understand about this test is that it measures a concentration, which means it reports the balance between sodium and water rather than the amount of salt in your diet. Patients almost always assume the opposite. A low sodium usually means there is too much water relative to sodium, not that you have been eating too little salt, and a high sodium usually means water has been lost or not replaced, not that you have been eating too much. The body defends this balance through thirst, through the hormone that tells the kidney to hold on to water, and through aldosterone, which governs how much sodium the kidney keeps.

    Both directions have real consequences, and because the brain is sensitive to the movement of water across its cell membranes, symptoms tend to be neurological: headache, nausea, confusion, unsteadiness, and in severe cases drowsiness or seizures. How quickly the change developed matters as much as how far it has gone, and correcting an abnormal sodium too rapidly can itself cause harm, which is why treatment is planned and supervised by a doctor rather than attempted with salt or water at home. Drink as you normally would: do not load up on water beforehand and do not arrive deliberately dehydrated, because either changes the very balance the test is measuring. A morning appointment is convenient but not essential. Bring a complete medicine list, mentioning diuretics in particular, along with antidepressants, some anti-epileptic and antipsychotic medicines, and any recent change in dose, as all of these are recognised influences on sodium.     Reading a High or Low Result

    What a higher result can point to

    • Water loss that outpaces intake, through fever, heavy sweating, vomiting or diarrhoea.
    • Reduced thirst or limited access to drinking water, which is more common in older or dependent people.
    • Diabetes insipidus, in which the kidney cannot concentrate urine and large volumes of dilute urine are passed.
    • Poorly controlled diabetes, where high blood glucose drives water out through the kidneys.
    • A large sodium load, usually from intravenous fluids given in a hospital setting.

    What a lower result can point to

    • Thiazide diuretics, among the most frequent causes seen in everyday practice.
    • Retaining water because of heart failure, liver cirrhosis or kidney disease.
    • Inappropriate release of the hormone that makes the kidney hold water, which can follow chest infections, some cancers, brain conditions and several medicines.
    • Drinking very large volumes of water or other low-sodium fluid over a short period, including during endurance exercise.
    • An underactive thyroid or adrenal insufficiency, both of which disturb water and sodium handling.

    These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests — your doctor decides what it means for you.

    What this test cannot tell you

    • The result describes the balance between sodium and water, so it says little about how much salt you eat and cannot be used to judge your diet.
    • Very high blood fats or protein can make sodium read falsely low on some methods, so an unexpected result is checked rather than acted on immediately.
    • A single value carries limited meaning without knowing how quickly it developed; the same figure is read differently in a sudden change than in a long-standing one.
