What the Potassium test tells you
Potassium is the principal mineral held inside your cells. The body works hard to keep far more of it inside cells than outside them, and that difference in concentration across the cell membrane is what allows nerves to fire and muscles to contract. The heart depends on this arrangement more visibly than anything else, because the electrical signal that coordinates each beat is generated by ions moving across membranes. A blood test measures the small fraction of potassium circulating outside the cells, and it is precisely because that fraction is small that modest changes in it matter.
The kidneys do most of the regulating, adjusting how much potassium is returned to the blood and how much leaves in urine, under the influence of the hormone aldosterone. Insulin and the acidity of the blood also shift potassium between the inside and outside of cells, which means a level can change without any potassium having entered or left the body. This is why the result is interpreted with kidney function, medicines, hydration and the clinical picture in view, rather than as a comment on how much potassium you eat.
Both directions carry weight. A high level tends to build quietly, most often when kidney filtration is reduced or when several medicines that conserve potassium are taken together, and it can disturb the heart's rhythm before causing any noticeable symptom. A low level more commonly follows fluid losses or diuretic treatment, and it shows itself as weakness, cramps and sometimes palpitations. Neither finding is a diagnosis on its own: your doctor will usually want to confirm an unexpected result, review your medicines and look at kidney function before deciding what it means.

What a higher result can point to
- Reduced kidney filtration, which limits how much potassium can be excreted.
- Medicines that conserve potassium, including ACE inhibitors, angiotensin receptor blockers and potassium-sparing diuretics.

- Poor intake combined with refeeding, when potassium moves rapidly back into cells.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests — your doctor decides what it means for you.
What this test cannot tell you
- A falsely high result is common and usually comes from the sample rather than the patient: repeated fist clenching, a tight or prolonged tourniquet, a difficult draw that damages red cells, or a delay before the sample is processed all release potassium into the specimen.
- Because of this, an unexpected high result is usually confirmed with a fresh, carefully taken sample before anything is concluded from it.
- The blood level describes only the fraction outside cells and is a poor guide to total body potassium, which can be depleted while the blood value still reads within the usual range.
