Electrolytes are minerals that carry an electrical charge when dissolved in body fluid. This panel reports the main ones together: sodium, potassium, chloride and bicarbonate. Sodium and chloride largely govern how much water your body holds and how it is distributed between blood and tissue. Potassium sits mostly inside cells and controls the electrical excitability of nerve, muscle and heart tissue, which is why the body defends it so tightly. Bicarbonate is the blood's principal buffer, absorbing acid produced by metabolism so that pH stays within the narrow window enzymes can work in.
The panel is read as a pattern rather than as four separate results. Sodium is usually a statement about water rather than salt: a low value most often means too much water relative to sodium, not too little dietary salt. Potassium moves with kidney function, acid-base state and a long list of medicines. Chloride generally tracks sodium, and when it does not, the mismatch itself suggests an acid-base disturbance. Bicarbonate falls when acid accumulates and rises with prolonged vomiting or diuretic use. Combinations of these shifts point towards particular causes far more precisely than any single value could.
Because the kidneys and hormones such as aldosterone and antidiuretic hormone do the regulating, this panel is also a window onto kidney function, adrenal function, hydration and drug effects. What a higher result can point to
- Water loss from dehydration, fever or reduced fluid intake, which concentrates sodium and chloride.
- Reduced kidney clearance, which allows potassium to accumulate.
- Potassium-sparing diuretics, ACE inhibitors, angiotensin receptor blockers and potassium supplements.
- Adrenal insufficiency, which characteristically raises potassium while lowering sodium.
- Prolonged vomiting or loop and thiazide diuretics, which raise bicarbonate as acid is lost.
- Red cells breaking open in the sample tube, which raises potassium in the report without any change in the body.
What a lower result can point to
- Excess water intake or fluid retention diluting sodium, as seen in heart failure, liver disease and inappropriate antidiuretic hormone secretion.
- Vomiting, diarrhoea or thiazide and loop diuretics, which strip potassium and chloride.
- Poor dietary intake, sustained alcohol use or the refeeding period after prolonged undernutrition.
- Uncontrolled diabetes with ketone production, kidney tubular disorders or severe illness, all of which consume bicarbonate.
- Hormonal causes such as excess aldosterone or cortisol, which drive potassium out of the blood.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests — your doctor decides what it means for you.
What this test cannot tell you
- Results are unusually sensitive to collection technique and to delay before processing, so an unexpected potassium is often repeated before it is acted on.
- The panel is a snapshot: values can shift substantially within hours as fluids, medicines and illness change.
- It shows that balance is disturbed but not why, so it is interpreted alongside kidney tests, medication review and the clinical picture.
