What the Tri-Iodothyronine, Free (FT3) test tells you
Triiodothyronine, or T3, is the thyroid hormone that does most of the work inside cells. It binds far more strongly than thyroxine to the receptors that switch thyroid-responsive genes on and off, so it is the hormone that actually sets the pace of metabolism in tissue. The free T3 test measures the small unbound fraction circulating in the blood — the part available to enter cells and act.
Most T3 is not secreted by the thyroid at all. It is made in the liver, kidneys, muscle and other tissues by removing an iodine atom from T4, a step each tissue can adjust to suit its own needs. That local control means free T3 reports on two things at once: how much hormone the thyroid is supplying, and how the body is choosing to convert it. It is the only routine thyroid test that says anything about the conversion step, which is why it is sometimes requested on its own rather than as part of a panel.
The same conversion step is also the source of this test's biggest pitfall. Serious illness, major surgery, starvation and prolonged calorie restriction all slow the conversion of T4 into T3 as a deliberate energy-conserving adaptation, and free T3 falls as a result. That fall is a normal response in an unwell body, not thyroid disease, and treating it as thyroid failure is a well-recognised misreading. Free T3 measured during an acute illness or shortly after an operation is therefore of limited value, and testing is generally better deferred until recovery.

What a higher result can point to
- An overactive thyroid gland, including Graves' disease and hormone-producing nodules.
- T3-predominant thyrotoxicosis, where T3 rises disproportionately to T4.
- Treatment with liothyronine or combination thyroid preparations containing T3.
- The early inflammatory phase of thyroiditis.
- Excessive iodine exposure in a susceptible thyroid gland.
What a lower result can point to
- An established underactive thyroid, usually after free T4 has already fallen.
- Serious non-thyroidal illness, major surgery or the period after severe trauma, when conversion of T4 to T3 is deliberately reduced.
- Prolonged fasting, severe calorie restriction or malnutrition.
- Drugs that block conversion of T4 to T3, including propranolol, amiodarone, corticosteroids and some contrast agents.
- Deficiency of selenium or other micronutrients that support the conversion enzymes.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests — your doctor decides what it means for you.
What this test cannot tell you
- Free T3 falls during serious illness, starvation and after surgery as a normal adaptive response, so a low value in those settings is easily misread as thyroid failure when the thyroid is not the problem.
- It is a poor test for detecting an underactive thyroid, because it stays in the usual range until the condition is well advanced.
- Assay methods differ between laboratories and results are not directly interchangeable.
- Biotin supplements can interfere with the measurement and give a misleading figure.
