What the Total Iron Binding Capacity (TIBC) test tells you
Total iron binding capacity describes how much iron your blood could carry if every available binding site were filled. Almost all of that capacity belongs to transferrin, the liver-made protein that ferries iron from the gut and from storage sites to the bone marrow, where it is built into haemoglobin. Measuring TIBC is therefore an indirect way of measuring how much transferrin is in circulation at the moment the sample was taken.
The body adjusts that capacity according to how much iron it can find. When iron is scarce, the liver produces more transferrin, so the measured capacity climbs — spare seats on an emptier train. When iron is plentiful, or when a chronic inflammatory illness is holding iron inside storage cells, transferrin production falls and the capacity drops. This is why TIBC often moves in the opposite direction to ferritin, and why the two together are far more informative than either one alone.
TIBC is rarely interpreted by itself. Laboratories combine it with serum iron to derive transferrin saturation — the proportion of the total capacity actually occupied — and that derived figure is usually what a clinician looks at first. Your doctor will read the pattern across iron, binding capacity, saturation and ferritin, alongside your blood count and your symptoms, rather than any single value in isolation.

What a higher result can point to

- Binding capacity reflects transferrin, not the size of iron stores, so it cannot replace ferritin.

- The result is a snapshot; a recent iron dose, infusion or transfusion can distort it.
