What the Thyroxine, Total (T4) test tells you
Total thyroxine measures every molecule of T4 in the sample — the small free fraction that can act on cells, plus the far larger portion travelling attached to carrier proteins in the blood. Because the bound portion dominates the figure, total T4 reports on carrier protein availability almost as much as it reports on thyroid output, and that dual nature governs how it is interpreted.
Three proteins do most of the carrying: thyroxine-binding globulin, transthyretin and albumin. Anything that raises them raises total T4 without the person being overactive — pregnancy, oestrogen-containing contraception or hormone therapy, and some inherited variants all do this. Anything that lowers them lowers total T4 without the person being underactive, including protein loss through the kidneys, chronic liver disease, malnutrition, androgens and high-dose corticosteroids. This single characteristic is why free T4 has largely replaced it for routine assessment.
Total T4 still has a place as a stand-alone measurement. It is robust and widely available, it remains the thyroxine value in longstanding follow-up records that were started with total hormone testing, and in certain situations — including assay interferences that distort free hormone measurements — the total value is the more reliable of the two. Whatever the reason for ordering it, the result is interpreted with TSH and with a clear picture of anything that might be altering carrier proteins.

What a higher result can point to
- An overactive thyroid gland, including Graves' disease and toxic nodules.
- Pregnancy, oestrogen-containing contraception or hormone therapy, all of which raise carrier proteins.
- Inherited excess of thyroxine-binding globulin, or familial variants that bind thyroxine unusually tightly.
- Excess thyroid hormone taken as medication.
- The hormone-releasing phase of thyroiditis.
What a lower result can point to
- An underactive thyroid from autoimmune disease, surgery or radioactive iodine.
- Loss of carrier proteins through the kidneys, as in nephrotic syndrome.
- Chronic liver disease or significant malnutrition, which reduce protein production.
- Androgens and high-dose corticosteroids, which lower thyroxine-binding globulin.
- Severe systemic illness.

What this test cannot tell you
- Total T4 cannot separate a genuine change in thyroid output from a change in carrier proteins.
- For that reason it is a weaker screening test than TSH or free T4, and is rarely interpreted alone.
- Drugs that displace thyroxine from its carriers can lower total T4 in someone whose thyroid is behaving normally.
- Results from different laboratory methods are not directly comparable, so trends are best followed in one laboratory.
