What the Testosterone, Free test tells you
Free testosterone is the small portion of the hormone that is circulating unattached to any carrier protein. Most testosterone in the blood is bound — tightly to SHBG and loosely to albumin — and bound hormone cannot readily leave the circulation to act on muscle, bone, brain or reproductive tissue. The free portion, together with the loosely albumin-bound part, is what tissues can actually use. This is why free testosterone is often described as the biologically active fraction, and why it can tell a different story from the total.
The divergence between free and total testosterone is the whole reason this test exists. When SHBG is high — with an overactive thyroid, oral oestrogen, liver disease, low body weight, or simply increasing age — more of the hormone is held in reserve and the free fraction can be low while the total looks unremarkable. When SHBG is low, as it commonly is with excess weight, insulin resistance or type 2 diabetes, the reverse happens and the free fraction holds up better than the total. Your doctor therefore reaches for free testosterone when the total result and the symptoms do not fit each other, or when SHBG is expected to be unusual.
How the free value is obtained matters. Most laboratories calculate it from the total testosterone, SHBG and albumin measured on the same sample; a directly measured free testosterone by equilibrium dialysis is regarded as the reference approach but is not used routinely. Different calculations do not always agree closely, so results are best followed within one laboratory. Testosterone also has a daily rhythm, running higher in the morning and easing later in the day, which is why the sample is conventionally taken in the morning and why a single low result is normally repeated on a separate morning before anything is concluded from it.
Preferred test when total testosterone is borderline, SHBG is expected to be abnormal, or symptoms of androgen deficiency persist despite normal total testosterone.
If SHBG is unusually high or low, the total can look normal while the active fraction does not — free testosterone resolves that mismatch.

What a higher result can point to
- Polycystic ovary syndrome in women, particularly where a low SHBG raises the active fraction.
- Testosterone therapy, anabolic steroid use or DHEA supplementation.
- Congenital adrenal hyperplasia and other adrenal disorders that increase androgen production.
- An androgen-producing tumour of the ovary or adrenal gland, which is uncommon but important to exclude.
- Low SHBG from excess weight or insulin resistance, which raises the free fraction without the total necessarily rising.
- Contamination of the sample from a topical testosterone gel applied to the skin before the blood draw.
What a lower result can point to
- Testicular failure, whether from a genetic cause, injury, infection, or treatment for cancer.
- A pituitary or hypothalamic problem that stops the signal reaching the testes, including tumours and pituitary damage.
- Excess body weight and insulin resistance, which lower testosterone through several routes.
- Long-term opioid painkillers, glucocorticoid treatment, or some hormone-suppressing cancer therapies.
- A raised SHBG from thyroid overactivity, liver disease or age, which reduces the free fraction even when the total holds up.
- Serious illness of any kind, which suppresses the reproductive hormone axis temporarily.

- Acute illness, poor sleep and significant stress temporarily lower testosterone, so testing is best deferred until you are well.
