What the Progesterone test tells you
Progesterone is the hormone of the second half of the menstrual cycle. It is made mainly by the corpus luteum, the temporary gland that forms from the follicle once it has released its egg. Its role is preparation: it converts the lining of the uterus from a growing tissue into a receptive one, quietens the muscle of the uterus, and holds those conditions steady in case a fertilised egg arrives. If no pregnancy occurs, the corpus luteum winds down, progesterone falls, and the lining is shed as a period. If pregnancy does occur, the corpus luteum continues until the placenta gradually takes over the job.
That biology explains why progesterone is measured the way it is. Before ovulation the level is naturally low, so a low result taken at that point tells you nothing at all. The measurement only becomes informative in the mid-luteal phase — the window roughly a week before the next period is expected — when a corpus luteum, if one formed, is at its most active. A well-timed sample in that window is one of the simplest ways to establish that ovulation actually happened in that particular cycle. A low result in a correctly timed sample suggests the cycle was anovulatory, meaning no egg was released that month, which can happen occasionally even in people who ovulate regularly.
Timing becomes harder when cycles are long, short or unpredictable, because the mid-luteal window has to be estimated backwards from a period that has not happened yet. In that situation your doctor may ask for more than one sample, or may time the test against ovulation tracking rather than the calendar. If your periods have stopped, or you are past menopause, the usual timing logic does not apply and your doctor will advise directly. What a higher result can point to
- Ordinary ovulation, with an active corpus luteum in the second half of the cycle — the expected reason for a high mid-luteal result.
- Pregnancy, in which progesterone rises steadily as the placenta takes over production.
- Progesterone given as treatment, including pessaries, injections, oral preparations and some contraceptives.
- A persistent corpus luteum cyst, which keeps producing the hormone after it would usually have regressed.
- Certain adrenal conditions in which precursors of progesterone accumulate and are measured alongside it.
- A rare ovarian tumour producing the hormone independently of the cycle.
What a lower result can point to
- A cycle in which no egg was released, so no corpus luteum formed — an occasional occurrence even for people who usually ovulate.
- A sample taken outside the mid-luteal window, which is a timing issue rather than a hormonal one.
- Suppression of the reproductive signal by very low body weight, intensive training, significant stress or illness.
- Raised prolactin or a thyroid disorder interfering with the ovulation signal.
- A corpus luteum that is not functioning well, sometimes in the setting of an early pregnancy that is not progressing, including an ectopic pregnancy.
- Menopause, where ovulation no longer occurs and low progesterone is expected.
These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests — your doctor decides what it means for you.
What this test cannot tell you
- The result is only interpretable if the sample was taken in the mid-luteal window, about a week before the next expected period; at any other point a low value is normal and means nothing.
- Cycles vary in length, so the window has to be estimated, and an unexpected result is often explained by timing rather than by a hormonal problem.
- Progesterone taken as medication raises the measured level regardless of whether ovulation occurred, so a result on treatment cannot confirm ovulation.
- Confirming ovulation is not the same as confirming fertility; it says nothing about egg quality, the fallopian tubes or the male partner.
