What the Insulin, Post Prandial test tells you
This test measures how much insulin is circulating two hours after an ordinary meal. Unlike a tolerance test, which uses a measured glucose solution, it works from the food you actually eat, so it describes your pancreas responding under the conditions of your everyday life rather than under laboratory conditions.
After a meal, blood sugar rises and the pancreas releases a surge of insulin to bring it back down. What interests your doctor is not the rise itself but the amount of hormone needed to manage it. When tissues respond poorly, far more insulin is required to clear the same amount of sugar, and that excess is the evidence of resistance. In some people the response is exaggerated or arrives late, pushing blood sugar lower than intended once the meal has been absorbed, which can explain shakiness, sweating and heavy fatigue some hours after eating.
The result is always read with the glucose drawn from the same sample, and often with a fasting insulin taken earlier the same morning. Because meals are not standardised, what you ate and exactly when you started eating are part of the interpretation, which is why you will be asked to record both. Insulin assays are also not standardised between laboratories, so results are most meaningful when compared within one laboratory over time. What this test cannot tell you
- Meals are not standardised, so what you ate changes the result and two visits are hard to compare directly.
- Timing matters: a sample taken early or late in relation to the meal describes a different point on the curve.
- Assay methods differ between laboratories, so results from different providers should not be placed side by side.
- In anyone already taking insulin, injected hormone cannot be told apart from their own, which is what C-peptide testing is for.
