    Creatinine, Serum

    Kidney Function Marker

Serum creatinine is a waste product from muscle metabolism filtered by the kidneys. This test is one of the most reliable markers for kidney function. No fasting is typically required.

    About This Test

What the Creatinine, Serum test tells you

Creatinine is a waste product formed when creatine phosphate, the rapid energy store inside muscle, is broken down. Because muscle turnover is fairly steady from day to day, creatinine is produced at a fairly constant rate and released into the bloodstream, where the kidneys filter it out and pass it into the urine. That steadiness is exactly what makes it useful. If filtration slows, creatinine has nowhere to go and begins to accumulate in the blood, which is why it is one of the most dependable chemistry markers of kidney function.

Interpreting it well means taking your body into account. Creatinine comes from muscle, so a heavily muscled person naturally sits higher than a small or frail person with identical kidney function. For this reason the laboratory converts creatinine into an estimated glomerular filtration rate using your age and sex, which puts the result on a common scale. Repeated measurements are also more informative than a single one: a clear rise from your own previous baseline can be significant even when the value itself still looks ordinary.

Creatinine also responds to things that have nothing to do with kidney disease. Dehydration, a large meat meal, an intense training session and creatine supplements all push it up, and some medicines block the small amount of creatinine the kidney tubules secrete without affecting filtration at all. Doctors therefore repeat the test, check hydration, review medication, and read creatinine alongside urea, electrolytes and a urine albumin measurement before concluding anything about your kidneys.

    Why Get Tested?

Fundamental test for assessing kidney function. Ordered routinely, before contrast dye procedures, when on nephrotoxic medications, or when kidney disease is suspected.

    Baseline kidney function

It is the standard first measure of how well the kidneys are clearing waste, and it establishes the baseline that later results are compared against.

    Safe medication dosing

Many medicines are cleared by the kidneys, and the dose depends on filtration, so this result guides safe prescribing.

    Monitoring known kidney disease

Serial creatinine shows whether kidney function is stable, improving or declining, which shapes treatment decisions.

    Checking before procedures

It is routinely measured before scans using contrast dye and before starting drugs known to stress the kidneys.

    When This Test Is Ordered

Reasons your doctor may request the Creatinine, Serum test

Your doctor may order this test for any of the reasons below. Some are routine checks, others follow specific symptoms or an earlier result.

    Routine kidney screening

As part of a routine health check or a metabolic panel.

    Symptoms of reduced kidney function

Symptoms such as ankle swelling, reduced urine output, nausea or persistent tiredness.

Risk factors, medication & known disease

Living with diabetes, high blood pressure or cardiovascular disease.

Before a scan using contrast dye or before starting a medicine that can affect the kidneys.

Monitoring established chronic kidney disease over time.

Adjusting the dose of medicines that the kidneys clear from the body.

    Preparation
    The short version
    No prep needed.

No fasting is needed for creatinine, so you can eat normally, but a few small things make the result easier to interpret. Avoid a large meat or offal meal and any unusually strenuous exercise the day before, since both push creatinine up temporarily. Drink fluids as you normally would; neither arriving dehydrated nor deliberately loading up on water is helpful. Tell the team if you take creatine or other sports supplements, as your doctor may ask you to pause them beforehand, and mention anti-inflammatory painkillers, antibiotics and any medicine started recently. Continue your prescribed medicines unless told otherwise. If this test is being repeated to follow a trend, try to give the sample at a similar time of day each time.

Ranges are a guide — your physician interprets your result alongside your symptoms and history.

Reading a High or Low Result

What a higher result can point to

    • Reduced kidney filtration, whether from acute kidney injury or chronic kidney disease.
    • Dehydration or any state that lowers blood flow to the kidneys, including severe infection and heart failure.
    • Obstruction of urine flow by stones, an enlarged prostate or a pelvic mass.
    • Medicines that damage kidney tissue, including some anti-inflammatories, antibiotics and contrast agents.
    • A large muscle mass, intense exercise, a heavy meat meal or creatine supplements, none of which indicate kidney damage.
    • Drugs that block creatinine secretion into the urine while leaving filtration untouched, such as trimethoprim and cimetidine.

What a lower result can point to

    • Low muscle mass, whether from a small build, frailty or advancing age.
    • Prolonged illness, undernutrition or advanced liver disease that reduces creatine production.
    • Pregnancy, which increases blood volume and kidney filtration.
    • Muscle-wasting conditions, long-term immobility or limb amputation.
    • A diet containing very little meat, which lowers the creatine available to muscle.

These are possibilities, not diagnoses. A single value outside the reference range is interpreted alongside your symptoms, history and other tests — your doctor decides what it means for you.

    Limitations & Safety

What this test cannot tell you

    • It reflects muscle as well as kidney function, so it overestimates filtration in frail people and underestimates it in very muscular ones.
    • It is insensitive early on: a substantial amount of filtering capacity can be lost before the value moves outside the usual range.
    • A single result without an earlier reading to compare with is difficult to interpret confidently.

    Risks of the blood draw

    • Mild bruising or brief soreness where the needle went in.
    • Occasional lightheadedness, which settles after a few minutes of rest.
    • Bleeding or infection at the site, which is rare with sterile single-use equipment.
    Key Points
    • 1Creatinine is a steady waste product of muscle metabolism cleared almost entirely by the kidneys.
    • 2It is the input the laboratory uses to calculate your estimated glomerular filtration rate.
    • 3Body build, hydration, exercise and diet all shift the result without any kidney problem being present.
    • 4A rise from your own baseline can matter even when the value still looks unremarkable.
    • 5It is interpreted with urea, electrolytes and a urine albumin test rather than in isolation.

    FAQ

    Frequently Asked Questions

Everything you need to know about the Creatinine, Serum test. Can't find what you're looking for?

    Talk to our team

Yes. Creatinine comes from muscle, so athletic and heavily built people often run higher than average with entirely healthy kidneys. Your doctor takes build into account rather than reading the number alone.

It can. When you are short of fluid, blood flow through the kidneys falls and creatinine rises even though the kidney tissue itself is undamaged. Such a rise usually settles once fluid balance is restored.

Not necessarily. Creatinine only moves once a considerable share of filtering capacity has been lost, which is why doctors also look at estimated filtration rate and at whether albumin is leaking into your urine.

Creatine supplements raise creatinine directly, and a very high protein or meat intake can nudge it up as well. Mention any supplements you use so the result is read in the right context.
