    What the Aspartate Aminotransferase (AST/SGOT) test tells you

    Aspartate aminotransferase, historically abbreviated SGOT, is an enzyme involved in amino acid metabolism. Unlike its partner ALT, it is not concentrated in the liver alone: substantial amounts sit in heart muscle, skeletal muscle, the kidneys and red blood cells. Any of those tissues can release it when damaged, which makes AST a broader signal of cell injury and a less liver-specific one.

    That breadth is exactly why AST is measured together with ALT rather than instead of it. Comparing the two lets your doctor infer where the enzyme is likely coming from and, within liver disease, which process is more probable. Alcohol-related liver injury tends to push AST proportionally higher than ALT, while metabolic fatty liver more often shows the reverse pattern, and advancing fibrosis can shift the balance again. None of this is diagnostic on its own — it is a clue that shapes what your doctor investigates next.

    Outside the liver, AST frequently rises for reasons that have nothing to do with it. Intense or unaccustomed exercise, particularly heavy resistance training, releases enzyme from skeletal muscle for several days. So does muscle injury, an intramuscular injection, a seizure, or a muscle-related side effect of some cholesterol medicines. Even the blood sample itself can be responsible: if red cells break during a difficult draw, AST leaks out of them and the reported value rises without anything being wrong with you. Mention any recent intramuscular injection, muscle injury, fall or seizure, and bring a full list of medicines and supplements, including cholesterol-lowering treatment and gym products. Staying well hydrated makes the blood draw easier, which reduces the chance of red cells breaking in the tube and falsely raising the result.

    • AST is present in several tissues, so a raised result does not confirm that the liver is the source.
    • A difficult blood draw that damages red cells can raise the reported value artificially.
    • Normal AST does not exclude significant liver disease, since advanced scarring often runs with unremarkable enzymes.
    • Recent exercise is a frequent and easily overlooked reason for an elevation in an otherwise healthy person.
