What the Anti-Thyroglobulin Antibody (Anti-TG) test tells you
Thyroglobulin is a large protein made and stored inside the thyroid gland; it is the scaffold on which thyroid hormones are assembled before they are released into the blood. This test looks for antibodies directed against that protein — a sign that the immune system has begun treating part of the thyroid as foreign. Their presence marks an autoimmune process rather than measuring how well or badly the gland is currently working.
Thyroglobulin antibodies are found in autoimmune thyroid disease, most often Hashimoto's thyroiditis and sometimes Graves' disease, and they are usually measured together with antibodies against thyroid peroxidase. They can also be present in people whose thyroid function is entirely stable, and they become more common with age, particularly in women. For that reason the antibody result is read as one strand of evidence beside your symptoms, your TSH and free hormone levels, and any ultrasound findings.
The test has a second and quite different use, and it is the most interesting thing about it. After treatment for thyroid cancer, thyroglobulin itself is measured as a marker of remaining thyroid tissue — and antibodies against thyroglobulin interfere with that measurement, often pushing the reported value misleadingly low. This is why the antibody is measured alongside thyroglobulin at every follow-up: not to assess autoimmunity, but to tell your doctor whether the thyroglobulin figure can be trusted. In that setting, a change in the antibody level over time can itself be informative.

What a higher result can point to
- Hashimoto's thyroiditis, the commonest autoimmune cause of an underactive thyroid.
- Graves' disease, where thyroglobulin antibodies often accompany the antibodies driving overactivity.
- Painless thyroiditis or postpartum thyroiditis.
- Autoimmune disease elsewhere, such as type 1 diabetes, coeliac disease or pernicious anaemia.
- Antibodies present without any thyroid dysfunction, which becomes more common with age.

What this test cannot tell you
- A negative result does not exclude autoimmune thyroid disease, since some people carry only peroxidase antibodies, and a negative or absent result has no established significance of its own.
- Antibodies can be present in people whose thyroid works normally and never causes trouble.
- The level does not track how underactive or overactive the gland is, so it is not used to adjust medication.
- Antibody methods differ between laboratories, so serial results are only comparable when measured in the same place.
