What the Alpha Fetoprotein (AFP) test tells you
Alpha-fetoprotein is a protein made in large quantities by the liver and yolk sac of a developing baby, where it plays a role comparable to that of albumin in an adult. Production falls away sharply after birth, and in adults only traces remain. Because the healthy adult liver has largely stopped making it, cells that revert to a more primitive state, as some liver cancers and germ-cell tumours do, can begin producing it again. That is what makes AFP useful, and it is also why it must be read carefully.
AFP is not a screening test for healthy people, and it should not be used to check for cancer in someone with no symptoms and no diagnosis. Its established roles are narrower. In people who already have cirrhosis or long-standing hepatitis B or C, AFP measured at intervals alongside ultrasound imaging is part of an organised surveillance programme. In germ-cell tumours of the testis or ovary, AFP together with beta hCG helps classify the tumour, judge how it is responding to chemotherapy, and detect recurrence. In both settings the trend across serial samples carries far more meaning than any single value.
A raised AFP very often has a harmless or non-cancerous explanation. In pregnancy AFP rises as a matter of course, and pregnancy values are interpreted against an entirely separate framework as part of screening for fetal conditions. Outside pregnancy, active hepatitis, cirrhosis and liver tissue regenerating after injury all push it up without any tumour being present. Equally important in the other direction, a normal AFP does not exclude cancer. A substantial share of liver cancers produce little or none of it, which is why surveillance pairs the blood test with imaging rather than relying on either alone. What a higher result can point to
- Pregnancy, where a rise is entirely normal and expected and is assessed against separate pregnancy criteria.
- Active hepatitis, cirrhosis, and liver tissue regenerating after injury, all of which raise AFP with no cancer present.
- Hepatocellular carcinoma, the commonest malignant cause in adults.
- Germ-cell tumours of the testis or ovary, particularly those containing yolk-sac elements.
- Cancer that has spread to the liver from elsewhere, in some cases.

What this test cannot tell you
- AFP is not a screening test for the general population; used in people without symptoms or a diagnosis it produces false alarms and unnecessary investigation.
- A normal AFP does not exclude liver cancer, because a substantial proportion of these tumours produce little or none of it.
- Non-cancerous liver disease and normal pregnancy both raise AFP, so a single raised value establishes nothing on its own.
- A low result in a non-pregnant adult is the expected finding and carries no independent significance.
